Pre/ Post Operative Instructions
Prior to your surgery, Mr Karametos will spend time with you to explain the procedure and complete Surgery Consent Form during consultation. Our office staff will then provide you with the general Pre/Post- Operative Instructions to take home. Please do read this document carefully before the surgery, you may also download a copy: pre___post_operative_instructions_[new].pdf . Hospital Admission Form
You can complete the hard copy which our staff provides and submit to hospital via mail or in person, Alternatively, you may complete this online. eAdmission forms need to be completed at least 72 hours prior to the procedure. Please follow the links below to complete your admissions form:
